The Distinguished Service Award is an internationally competitive award granted annually to a maximum of 50 Rotarians who have exhibited exceptional active service to The Rotary Foundation. Financial contributions to the Foundation, however notable, are not relevant to this award.
Nominees must have received the Citation for Meritorious Service at least four years prior.  Any active Rotarian in good standing may be nominated.  Previous recipients of the Distinguished Service Award are not eligible.  You may wish to check the Distinguished Service Award recipient list before submitting  your nomination.  You may not nominate yourself.
You will be asked to provide information about the individual and his / her service activities and accomplishments. You may wish to download the Distinguished Service Award Nomination Form before entering the achievements on this form.
